Clinical Tours Spring 2015

A minimum of six (6) clinical site tours comprise SPP I Spring 2015. A site report is required for each of the six site tours. Grading criteria provided.

1. Site Report must be submitted in a word processed format. Report will be written in a narrative writing style, double spaced using 1 inch margins and a suitable typeface such as Times New Roman. Your report should be written in 12 point font. Spelling and grammar will count. The report itself should not exceed six to eight total pages in length.

2. Required: Facility name, date of attendance, and site supervisor name is required on each report.

Points will be awarded in the following manner:

Report Content (Are the Questions Addressed?) 30 points ( 6 reports /5 pts ea)

Grammar and Spelling 5 points

Format (margins, spacing, font, etc.) 5 points
